Kroger's pharmacy will be doing our flu shot clinic for us on Monday, October 11th (time to be announced). Please sign up in the clinic. You will be given a vaccine consent form to fill out and bring with you on Monday. You will also need to bring a copy of your prescription insurance card (this is your Caremark card if you have school health insurance). Contact Nurse Jill with any questions.
